Patient Participation Group (PPG)

Patients and practices work together to help patients to take responsibility for their health and to provide strategic input and advice.  This is a forum for patients and staff to improve and develop services within the practice by co-production. It allows the practice to be closer to the community that they serve.  It usually meets three monthly.  The PPG also hosts education and awareness events & may help the practice with achieving objectives.  If you would like to join the group or put a question to the group there is a comments and suggestions box. On Thursday 4th August 2022, we held our PPG meeting. Below, you can find out what was discussed at the most current meeting, as well as our previous meetings.

 

Community Health and Wellbeing workers (CHWW)

You may start to see our new Community Health and Wellbeing Workers around the surgery. They are working collaboratively with Millbank Medical Centre to promote the health and wellbeing of local individuals and groups. The team will mainly be out and about in the community visiting patients at home to support them with their health and care needs.

It’s a new initiative that provides proactive outreach to the community in Churchill Gardens – at the moment we only have 4 part time CHWWs who cover around 500 households in the area, visiting households regularly and building trust and relationships to increase health literacy, uptake of prevention and screening opportunities, light social prescribing and chronic disease support as well as helping with housing, employment and other wider determinants.

Data Sharing

Since 1st September 2021 your data has been shared with NHS Digital to help improve health, care and services.

Patient data from GP medical records kept by GP Practices in England is used every day to improve health, care and services through planning and research, helping to find better treatments and improve patient care.

The NHS is introducing an improved a way to share this information – The General Practice Data for Planning and Research data collection.

NHS Digital will collect, analyse, publish and share this patient data to improve health and care services for everyone.

This includes:

  • Informing and developing health and social care policy
  • Planning and commissioning health and care services
  • Taking steps to protect public health
  • In exceptional circumstances providing you with individual care
  • Enabling healthcare and scientific research

Any data that NHS digital collect will only be used for health and care purposes. The data will never be shared with marketing or insurance companies.
