At some point we will all have health and wellbeing needs, these may include support regarding diet, exercise, social interactivity and mental health.

Millbank Medical Centre is delighted to be working in collaboration with Westminster City Council to provide a team of Community Health and Wellbeing workers (CHWWs) to residents of Churchill Gardens.
Frequently asked questions
Q: How often will I meet with a Health & Wellbeing worker?
A: Each patient participating in the scheme will be visited in person on a monthly basis.
Q: How will I be sure the individual visiting me is who they say they are?
A: All Health & Wellbeing Workers carry NHS and Westminster Council ID cards. If you feel unsure you can contact Millbank Medical Centre and verify the induvial
Q: what can they help with?
A: sign posting to local and national, services that will benefit, both physical and mental wellbeing
Q: What if I am not ready to be a part of this programme at present?
A: If individuals feel now is not the right time to engage, they can contact Millbank medical Centre at a later date and ask to be enrolled on the programme and a member of the Health & Wellbeing Team will be in contact.
If you have been assigned Community Health and Wellbeing Worker, you will receive a letter from Millbank Medical Centre to introduce them. The CHWW will then contact you arrange a date an d time to visit you at home.
